Each year, almost 10,000 serious workplace injuries and more than 25,000 serious injuries from road crashes are caused by poor alertness. This results in around $5 billion in lost productivity each year.
Driver sleepiness caused two in 10 serious car crash injuries in Australia, said Monash University Professor Shantha Rajaratnam, the project’s research leader.
He believed a roadside test for drowsy drivers was achievable and envisages that new devices that could test workers’ sweat or saliva. ‘‘It is something that law enforcement agencies all over the world have been looking for.’’
Getting a good night’s sleep was not as simple as it sounded, said Bob Baldwin, Member for Paterson and parliamentary secretary to the minister for industry. Yesterday he launched a new co-operative research centre for alertness, safety and productivity at Monash.
The new centre will bring together a team from 26 universities, tech companies and government agencies. The federal government is investing $14.5million in the centre over seven years.
MP Bob Baldwin said that ‘‘Over 20 percent of the population are affected by a sleep disorder’’.

The American College of Physicians (ACP) review of clinical evidence found that because of it’s effectiveness, CPAP therapy remains the best first option for treating sleep apnea after an initial diagnosis. Unfortunately, some patients don’t use CPAP therapy once prescribed.
Practice, perseverance and peer support are critical to success, with discomfort of CPAP masks being the main reason why patients abandon their CPAP treatment. Despite this, CPAP therapy is the most effective treatment for sleep apnea.
In Australia, CPAP therapy has been very expensive due to the market dominance of Resmed, Philips Respironics and F&P. Typical prices from these companies for CPAP Machines have been $1500 and $2,500 for the Auto (APAP) machines.

A secondary alternative to CPAP therapy is known as Mandibular Advancement Devices (MAD) where a mouth piece is custom made and designed to pull the lower jaw forward in an attempt to increase upper airway respiration.
This can be effective in treating sleep apnea, but success is not guaranteed.
The cost of such an oral device is painful to the jaw and pocket. Around one third of patients need to replace their MAD within three years, further adding to their cost of CPAP therapy. Additional costs and the inconvenience of frequent dental visits also need to be considered with this MAD form of CPAP therapy.
Another alternative to CPAP therapy (not recommended by the ACP) is the Provent device.
The concept of this device is to block most of the nasal expiration effort with disposable strips of special tape over the nostrils.This creates a positive pressure upon exhalation, if the patient breathes through their nose.
This is a costly treatment, typically more than $1,300 per year in Australia, and is ineffective if the patient opens their mouth to breath.
Weight loss can have a positive impact on obstructive sleep apnea (OSA).
According to the ACP report, rates of OSA are on the rise, as are obesity and other weight-influenced diseases such as Type 2 diabetes. Being overweight is one of the most significant risk factors for developing obstructive sleep apnea. The ACP’s first recommendation to doctors is that they encourage their overweight and obese patients with sleep apnea to lose weight.
A combined approach of initial CPAP therapy, exercise and reduced calorie intake supported and encouraged by associated health care professionals and peers support is the best recipe for success.

CPAP machines put out a fixed (constant) pressure based on a sleep study and doctor’s prescription. This is the minimum pressure required to keep the airway open during sleep.
Even if you are still awake, the machine will still put out this pressure.
If you do not know the treatment pressure you need, we can’t supply a cpap machine as we don’t know what to set it to.
The auto machine will adjust up or down as required between a pre-set range. So if you are still awake, the machine will put out a lower pressure and only increase when needed during sleep. Also the pressure required is higher when sleeping on your back. The auto machine will cope with these changes by varying the pressure automatically.
“In my personal opinion, this is more comfortable and smarter way of providing the required therapy.”
In the future, the treatment pressure required may change due to weight gain or weight loss, ageing, loss of muscle tone in the throat, etc. The auto machine will keep adjusting automatically to the correct pressure, often without the need for another sleep study.
Imagine you have the choice of a car, manual or automatic.
A manual car with only one gear (is there such a thing these days?) is not the most efficient or smart way to drive.
An automatic car will select the correct gear. An auto machine will select the correct pressure all the time. Easy.
In my opinion, it is the same with cpap therapy, Auto (APAP) is better than fixed (CPAP) pressure for many reasons.
